Patient Registration

Patients wishing to register with us are asked to complete a registration form and a new patient questionnaire. These forms are available to collect at our reception desk during practice opening hours. Alternatively, please download, print and complete the following two forms and bring them to our reception. They cannot be emailed or faxed to us.

When registering with us we will need to see photographic proof of your identity e.g. your passport / driving licence and proof of your current address e.g a recent bank statement / utility bill. New patients are invited to book an appointment with one of our health care assistants for a new patient health medical. Temporary Registrations

If you are ill while away from home or if you are not registered with a doctor but need to see one you can receive emergency treatment from the local GP practice for 14 days. After 14 days you will need to register as a temporary or permanent patient.

You can be registered as a temporary patient for up to three months. This will allow you to be on the local practice list and still remain a patient of your permanent GP. After three months you will have to re-register as a temporary patient or permanently register with that practice.

To register as a temporary patient simply contact the local practice you wish to use. Practices do not have to accept you as a temporary patient although they do have an obligation to offer emergency treatment. You cannot register as a temporary patient at a practice in the town or area where you are already registered.

Practice Boundary

Before you register with our practice you need to check that you live within out practice boundary. If you live outside our practice boundary you may still register with us although you will not be eligible for home visits and will be asked to sign a disclaimer confirming this. Our practice catchment area covers Churchrown and surrounding areas.

The practice area covers postcode PR9 – from Saunders Street/Manchester Road/Tithebarn Road/Crowland Street/Cobden Road/High Park Road/Moss Lane/Water Lane/Banks Road/Coastal Road to Sandpipers/Promenade.
